Click on PD Models on the left and choose the model you want. You can fill it out on-line and submit it right to Jean Jennings. We urge you print out and/or read the entire application before filling it out if this is your first time.

Once your application is received, it will be reviewed and you will be notified of approval or needed additions or adjustments.

Upon approval, your learning opportunity (except when you are notified otherwise) will be posted on the registration system for your participants to register.
